TERMS & CONDITIONS OF REGISTRATION AND PURCHASE

This Agreement includes:


Ownership

The registrant of a Continuing Education (CE) Activity must be a licensed pharmacist, or a licensed medical professional, or working directly under the supervision of a licensed pharmacist. LP3 Network Inc. reserves the right to inquire about, or request, a written copy of the qualifications of the registrant of a CE Activity. The registrant of a CE Activity represents and warrants that they are competent, qualified and hold the appropriate credentials, and will make such credentials available to LP3 Network Inc. upon request.

LP3 Network Inc. explicitly reserves the right to dismiss a registrant from participation in an Activity for violation of adherence to professional standards of pharmacy practice and the purchaser will not be entitled to any refund for a purchased Activity.

All Activity materials are © Copyright LP3 Network Inc. LP3 Network Inc. has taken significant measures to protect its property, under the law, by applying trademarks and copyright to its products and services. No trademarked or copyright material or any other content may be distributed, downloaded, modified, reused, reproduced, reposted, retransmitted, disseminated, sold, published, broadcasted, circulated or otherwise used in any way whatsoever except as expressly stated, without the express written consent of LP3 Network Inc.

It is strictly forbidden to use the name LP3 Network Inc., or its official logo, for the purposes of business development and marketing without the express written consent of LP3 Network Inc. LP3 Network Inc. reserves the right to restrict any other unauthorized use of its name without express written consent.


Disclaimer

Please read the definitions that are frequently referred to in this Agreement and through the web site.

All Continuing Education Activity material is for educational purposes only as part of an educational Activity presented and accredited by a recognized and accredited provider of continuing education, and in accordance with the provider’s standards. LP3 Network Inc. is obligated to adhere to the policies of its associated accredited providers under their respective guidelines.

Continuing Education Activity material may include treatment modalities, diagnostic and therapeutic information, and is for educational reference purposes only, and should not be taken as a treatment regimen, product indication, or suggested treatment modality. Any treatments or therapy must be fully investigated and prescribed only by a duly licensed medical practitioner in accordance with accepted professional standards and compendia. Pharmacists, or any other health professional, are advised that Activity material should not be used as a means of establishing therapeutic interventions; rather therapeutic interventions should be based on a prescriber's independent clinical judgment. No claims are being made as to the safety or clinical efficacy of any suggested treatment interventions. LP3 Network Inc. cannot be held liable to any person or entity concerning claims, loss, or damage caused by, or alleged to be caused by, directly or indirectly, the use or misuse of the information contained in any Activity material.

LP3 Network may revise and/or modify its Content contained within, and at any time. The purpose of such revisions and/or modifications is to make improvements and to ensure the highest possible quality assurance. LP3 Network encourages the registrant to check the content of this Agreement periodically for any updates and/or Content changes.


Credit Policy

In accordance with current policy, this Activity must be completed in its entirety in order to receive credit of completion; partial credit for partial completion is no longer offered.


Payment

Payment for a CE Activity must be made by means of an approved credit card and received in full prior to the receipt of any Activity material. Activity prices are quoted in US Dollars [USD] and are subject to change.


Cancellation and Refund Policy

In the event that LP3 Network cancels a course, LP3 Network's sole liability shall be course repayment. In no event shall LP3 Network be responsible for any costs related to travel and/or accommodation.

A written request must be sent to LP3 Network Inc. requesting ‘Cancellation without Transfer’ to an alternate live activity event date. If cancellation occurs at greater than or equal to 31 days from live activity event date, then registrant will receive a 75% refund; 15-30 days a 50% refund; and less than or equal to 14 days no refund. There are no refunds, returns, or transfer requests upon purchase of a home study, digital work book, webinar, and elearning modules.

LP3 NETWORK INC. shall be excused from any delay caused by reason of any occurrence or contingency beyond its reasonable control (a “Force Majeure”), including but not limited to, acts of God, hurricane, earthquake, labour disputes, strikes, riots, war, and governmental requirements. The obligation to pay money to LP3 NETWORK INC. in a timely manner is absolute and shall not be subject to this Force Majeure provision. In such event, LP3 NETWORK INC. will not issue any refunds, only transfer to an alternate live activity event date.


Transfer Policy

Transfer policy is only in effect if requested greater than or equal to 14 days prior to original live activity event date. Failure to submit "Transfer Request" at least 14 days prior to original live activity event date will default to "Cancellation and Refund Policy". Transfer policy can only apply one (1) time. Transfer to a new live activity event date must be within current calendar year. Failure to attend the new live activity event will lead to no refund and will no longer be transferable. Confirmation of new activity date is subject to the approval by LP3 Network Inc. based on the number of available seats within the selected live activity event. There are no refunds, returns, or transfer requests upon purchase of a home study, digital work book, webinar, and elearning modules.


Practical Laboratory Liability Policy

The registrant understands that he/she is participating in training which may involve medicines, chemicals, laboratory equipment and other materials related to the training which may present a hazard if misused or used improperly or not in accord with reasonable safety procedures. If registrant is pregnant, has a medical condition or disease or has recently suffered an illness or injury, registrant should have or did consult her physician before participating. By agreeing to these terms, the undersigned hereby acknowledges their responsibility to exercise the highest standards of safety and caution in the handling and use of these materials and agrees to be responsible for, and hereby releases and discharges LP3 Network Inc., their directors, officers and employees, heirs, executors, administrators, agents, assigns and successors, and all other persons, firms, or who might be claimed to be liable, from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, and particularly on account of all injuries, known and unknown, both to person and property, which may result from the negligence of the registrant in the handling or use of any such materials.

The undersigned hereby declares that the terms of this release of claim with LP3 Network Inc. has been completely read and is fully understood and voluntarily accepted for the purpose of making full and final release of any and all claims, disputed or otherwise, on account of the injuries and damages above mentioned, and for the express purpose of forever barring any future claims arising therefrom.


Marketing Consent and Authorization

I hereby give my consent to MEDISCA®Inc., MEDISCA® Pharmaceutique Inc, MEDISCA® Network Inc.,MEDISCA® Australia Pty Ltd, MEDISCA® UK Ltd., and LP3 Network Inc. (hereinafter “MEDISCA and/or LP3”), and all persons acting under its authority, to use, in whole or in part, my name, photograph, video or voice record and my image, in its publications, advertising or other media activities (including the Internet). This consent includes, but is not limited to:

  • Permission to MEDISCA and/or LP3 to interview, film, photograph, tape, or otherwise make a video production of me and/or record my voice;
  • Permission to MEDISCA and/or LP3 to record the Live Program on which I appear for educational, internal, promotional, marketing and/or archival purposes;
  • Permission to use my name; and
  • Permission to use quotes from the video or testimonial(s) (or excerpts of such quotes), the photograph(s), tape(s) or reproduction(s) of me, and/or recording of my voice, in part or in whole, in its publications, other print media, and electronic media (including the Internet).

I acknowledge that MEDISCA and/or LP3 own all rights and privileges to the images, recordings and testimonials including all reproductions thereof.

I hereby waive:

  • Any right to inspect or approve the use of the images, recordings and testimonials or of any written copy;
  • All copyright and moral rights; and
  • Any right to royalties or other compensation arising from or related to the use of the images, recordings or testimonials.
